Radio System Administrative Forms and Documents

Radio Subscriber Change Control Form -This form should be submitted by the Radio Management Administrator of an agency desiring a radio to be reprogrammed with a new trunked ID or template and/or a new console alias be assigned.

Request for Cache Radios - This form should be submitted by an authorized Berks County public safety agency needing to borrow additional radios to meet some temporary public safety need in their community. It should be used only after all other means to meet the need have been exhausted.
